How do they work?
Semaglutide (Ozempic and Wegov): Belongs to a class of medications called GLP-1 receptor agonists. Semaglutide works by mimicking the action of a naturally occurring hormone called glucagon-like peptide-1 (GLP-1), which helps regulate blood sugar levels, appetite, and food intake. By activating GLP-1 receptors, semaglutide slows down digestion, reduces appetite, and promotes feelings of fullness.
Tirzepatide: (Mounjaro and Zepbound) Tirzepatide is a dual glucose-dependent insulinotropic polypeptide (GIP) and GLP-1 receptor agonist. It activates both the GLP-1 and GIP receptors, which leads to increased insulin secretion, reduced glucagon secretion (which lowers blood sugar levels), delayed gastric emptying, and decreased appetite. Tirzepatide's dual agonism is believed to provide enhanced efficacy in blood sugar control and weight loss compared to selective GLP-1 receptor agonists like semaglutide.
Who cannot take these medications?
While GLP-1 receptor agonists (GLP-1s) are generally well-tolerated and effective for many individuals, there are some absolute contraindications that might prevent their use.
These contraindications typically include:
1. History of Severe Gastrointestinal Disease: Patients with a history of severe gastrointestinal diseases, such as gastroparesis or pancreatitis, may have an increased risk of complications and are usually advised against using GLP-1s.
2. Medullary Thyroid Carcinoma (MTC) or Family History of MTC: GLP-1s might not be recommended for individuals with a personal or family history of medullary thyroid carcinoma due to the potential risk of exacerbating this condition.
3. Type 1 Diabetes: GLP-1s are primarily indicated for type 2 diabetes and are not typically prescribed for individuals with type 1 diabetes.
4. Pregnancy and Breastfeeding: There is limited data on the safety of GLP-1s during pregnancy and breastfeeding, so caution is advised, and their use in these situations is generally avoided.
5. Severe Renal Impairment: In cases of severe renal impairment or end-stage renal disease, dose adjustments or avoidance of GLP-1s might be necessary due to decreased excretion of the drug.
6. Severe Hypersensitivity or Allergy: Individuals with known severe hypersensitivity reactions to GLP-1 receptor agonists or their components should avoid these medications.
